  • HOW TO PROCEED IF YOU HAVE LOST YOUR POLISH PASSPORT DOCUMENT OR IDENTITY CARD?

  • If you have lost your Polish passport document (i.e. your passport document was lost, stolen or damaged) you are under the obligation to report your loss to passport authority (issuer of the document or the Consular Section of the Polish Embassy in The Hague).

     

    If your passport document has been stolen and you are victim of criminal offence we recommend you to report  to a local Police station in the Netherlands.

     


     

    How can I make a notification on loss of my passport document

     

     

    • Arrive personally to the Consular Section of the Polish Embassy in The Hague (Alexanderstraat 25, 2514 JM Den Haag)
    • Fill in and sign a notification form
    • On the spot the consular officer will review your notification. Subsequently, your lost passport document will be marked as annulled in the central passport evidence.  This operation is irreversible. Even if afterwards you will find your passport document it would not be valid any more.

    Passport document reported lost which have been subsequently found (by its holder or a third person) would not be return by the passport authority. If you have found your passport document that was previously notified  lost, you are obliged to return it to the nearest Polish passport authority.

     

    Important!

    Please note that crossing foreign borders with passport document that was reported lost is not allowed. Take it into consideration while planning your foreign trips to avoid serious inconveniences.

     


     

    Loss of a Polish Identity Card (dowód osobisty)

     

     

    If you have lost your Polish Identity Card, it was damaged or stolen at the territory of The Kingdom of The Netherlands, you are obliged to notify the Polish Consulate in The Hague immediately.

     

    To make a notification:

    - fill in the form (download the form)

    - submit the form to the Consular Section of the Polish Embassy in Hague (Alexanderstraat 25. 2514 JM Den Haag)

    • in person, while visiting the Consular Section (no appointment needed);
    • by post, sending duly fill in and signed notification form, or
    • by fax transmission (+31 (0) 70 79 90 147), sending duly fill in and signed notification.

     

    Please note that it is not admissible to notify the loss or damage of your ID by e-mail.

     

     

    Important:

    If you are the user of the co called Trusted Profile (Profil Zaufany) you can also make notification of loss or damage of your ID via e-PUAP. Please see more: https://obywatel.gov.pl/

     

    It is not possible to make an application for a new Polish Identity Cart in the Consular Section.

    Consuls do not issue or deliver Polish Identity Cards.

     

     


     

    Loss of Polish driving license or car registration certificate

     

     

    In case of loss of your Polish driving license or Polish car registration certificate please contact your document issuer.

    The Consular Section in The Hague  is not competent to receive and proceed your notification on loss of Polish driving license or registration certificate.

    If your driving license document, registration certificate or registration plates are stolen and you are victim of criminal offence we recommend you to report  to a local Police station in The Netherlands.
