  • HOW TO APPLY FOR A POLISH PASSPORT DOCUMENT?

  • First - make an appointment:

     

    All applicants seeking appointments in passport-related matters can be accommodated only after online registration at: www.e-konsulat.gov.pl

     

    while you are at : www.e-konsulat.gov.pl

    • choose the language version: English
    • select country: Netherlands
    • select diplomatic mission: The Hague
    • at Menu select purpose of your visit: passport matters
    • book your appointment  by indicating the number of passport applicants and available/suitable date and hour
    • fill in the registration form with personal details
    • download confirmation of your visit and print it out or save it on your phone. You are expected to present this confirmation on the day of your appointment.

     

     

    If you are an adult and wish to make an application for a Polish passport valid for 10 years:

     

    • You have to appear in person at the Consular Section of the Polish Embassy in The Hague (Alexanderstraat 25, 2514 JM Den Haag). It is not allowed to use third parties or apply by mail.
      • Fill in passport application form using block letters and then sign it. Application form is available only at the consular office. The form is not available for download due to the required specification of paper weight and its size.
    • Other requirements:
      • one passport photo (passport photo requirements - see below);
      • valid Polish passport or ID card (dowód osobisty). If its validity has expired, latest possessed Polish identification document;
      • If submitted documents give rise to doubts as to the applicant’s identity or Polish citizenship, the Consul may request additional documents, in particular excerpts from Polish civil status register (birth and marriage certificates) or decision by relevant Voivode in Poland  confirming Polish citizenship status of the applicant;
      • payment of consular fee;

    You can pay the fee on the day of the visit by cash or Dutch bank card, or beforehand by bank transfer for the Consular Section’s account number;

    • Waiting time for your new passport will be 4 - 5 weeks. You must appear in person to collect the document. In justified cases, at your written request submitted together with your passport application the Consul may waive the requirement to personally collect the passport. In such an instance an extra payment of 10 EUR for delivery fee will be charged.
    • You can track personalization status of your passport on the website https://obywatel.gov.pl/wyjazd-za-granice/sprawdz-czy-twoj-paszport-jest-gotowy by providing the 16 digit number of you application, which you can find under the bar code of  the confirmation of approval of your application.

      In emergency situations, in particular when you lost your Polish identification document while staying in The Netherlands and there is an urgent need to travel back to Poland or to a country of your residence the Consul may provide you with a temporary passport (with short term validity). Requirements for temporary passports are analogous to those applicable to passports valid for 10 years.  

       

       

    If you apply for a passport for your minor child (under the age of 18 years):

     

     

    • Make an appointment (see above) at the Consular Section of the Polish Embassy in The Hague (Alexanderstraat 25, 25 14 JM Den Haag)
    • If you are parents/legal guardians you both have to appear in person, it is not possible to apply via third parties or by mail.
    • Minor over the age of 5 is required to be present at the consular office together with his/her parents/legal guardians while submitting passport application. Children below age of 5 are waived from requirement of personal presence.
    • Fill in passport application form using block letters and then sign it. Application form is available only at the consular office. The form is not available for download due to the required specification of paper weight and its size.
    • Other requirements :

     

    • one passport photo (passport photo requirements - see below);

    • valid Polish passport or ID card (dowód osobisty) of the minor. If its validity has expired, latest possessed Polish identification document. If the child never had a Polish identification document, his/her Polish birth certificate (also if the minor was born abroad).

    • valid passport or ID card (dowód osobisty) of both parents or legal guardians;

    • If one parent/legal guardian is unable to appear in person in the consular office on the day of submitting passport application his/her written consent for granting passport application must be delivered. Parent/guardian signature on the document must be legalized by the Polish passport authority or by public notary;

      • If documents submitted give rise to doubts as to identity or Polish citizenship of the applicant, the Consul may request additional documents, in particular excerpts from Polish civil status register (birth and marriage certificates) or decision by relevant Voivode in Poland  confirming Polish citizenship status of the applicant;

    PASSPORT PHOTO REQUREMENTS
     

    Very recent taken within the last 6 months with neutral face expression, done in front (en face), with closed mouth, with open eyes, without a hat or dark glasses, face not covered with hair.

    Color with high quality paper, clear white/off-white background.
